Not really his fault (if it's the one I'm thinking of). McNabb placed the ball way to high for Buckhalter. He put it almost at his chest/neck instead of down lower.

There was that one, plus another one which was all Buckhalter's fault. A.J. Hawk hit him and he lost the ball as he was falling forward inside the five yard line.
