Constantly.  What kills me is there is no improvement or reduction in the continues mistakes...The same things over and over and over...No way the staff even pays attention in practice.

Its been there.  Can't remember when they started but it definitely wasn't there at the beginning of the year.  Maybe something after the bye week?
