    Received a tax form from Coventry. On the bottom it lists my name and last 4 of SSN, but for some reason lists another guys name and SSN that I do not know. Called, gal says it's an ongoing issue. Tried telling me my name and last 4 of SSN wouldn't be enough to steal my identity...

      All it takes for some credit lines is a last 4 of SSN and DOB. Contact equifax and have a free 30 day credit freeze put on just in case.
