His entire schtick is telling rural Americans that the America they know is being destroyed, the country is in ruin, crime is seeping into their neighborhoods, etc. His speech about American carnage epitomizes this ethos and galvanizes his base - a horde of uneducated rural Americans. 

 

Those same voters, of course, have a well documented history of voting against their own interests. Simultaneously, rural areas are bleeding people, wealth, and their tax base. This problem is not unique to America, and I'm not sure what policies we can realistically pass to improve the situation.
